Best Mattress for Back Pain: Where Five Review Sites Agree and Differ
Four of the five review sites compared here put the Helix Midnight Luxe first on their back-pain or orthopedic list. The feel they point to is medium-firm, a bed that keeps your spine in a fairly straight line.

For a back pain best mattress shortlist, 5 publishers run 6 lists, and 3 put the Helix Midnight Luxe first. A fourth, Sleep Advisor, picks the Helix Midnight with ErgoAlign. ErgoAlign is an add-on layer built to support the lower back.
Shop for a medium-firm feel, a bed that keeps your spine level. Which bed fits you depends on your budget and how you sleep.
Check this first. A mattress is not the answer to every kind of back pain. If your back pain is severe, Mattress Clarity says to see a medical professional, so do that before you shop. Our guide on how to tell back pain from organ pain covers the signs to check.
Your Mattress Can Add to Back Pain, but It Is One Cause of Many
Mattress Clarity says back pain has many causes, but an uncomfortable, unsupportive bed “can be a large contributor” (Mattress Clarity). Naplab goes further: it says an unsupportive mattress can make pain you already have worse, or cause new pain altogether.
Neither site says a new bed will end your pain. What they’re saying is that a bed with poor support is one factor worth ruling out.

Support Means a Level Spine and a Medium-Firm Feel, Around 6 Out of 10
Sleep Advisor defines neutral alignment as your spine lying in a fairly straight line, “with no odd dips or unnatural curves” (Sleep Advisor). In plain terms, a supportive bed holds that line. It doesn’t let your heaviest part sink.
On firmness, Sleep Advisor recommends a medium-firm to firm mattress for people with back pain. Its comparison table rates the Helix Midnight “Medium-Firm: 6/10”. Naplab only lists beds rated 6 to 7 out of 10 (Naplab). NCOA’s back-pain list says it focused on medium-firm models that can help keep the spine aligned.
“Orthopedic” Is a Marketing Word With No Formal Definition
The National Council on Aging (NCOA) notes that “orthopedic mattress” has no formal definition. In general use, the term means a bed designed to keep your spine aligned and spread your weight evenly.
So the label describes a goal. It isn’t a standard a bed has to pass. The useful check is the same as above: does the bed keep your spine level at the firmness you need? A bed sold without the word can meet that just as well.
Helix Midnight Models Come First on 4 of 6 Lists
How the count works: a “top pick” is the bed a page names first or gives its overall-winner label. A category pick, such as NCOA’s lower-back-pain label, is counted separately. NCOA publishes two lists, one for back pain and one for orthopedic beds, so five publishers give six lists. Rows where the Helix is the top pick come first.
| Publisher and list | Top pick, as labeled | Where the Helix Midnight sits | Firmness advised | Price shown (as listed Oct 2026) |
|---|---|---|---|---|
| AARP, back pain | Helix Midnight Luxe, “Our Top Mattress for Back Pain” | Top pick (Luxe) | — | — |
| Mattress Clarity, back pain | Helix Midnight Luxe, “Clarity’s Clear Winner” | Top pick (Luxe, ErgoAlign layer optional) | — | — |
| NCOA, orthopedic | Helix Midnight Luxe, NCOA’s best overall | Top pick (Luxe) | — | Queen range $998–$3,499 |
| Sleep Advisor, back pain | Helix Midnight with ErgoAlign Layer, Editor’s Pick | Top pick (Midnight, not Luxe) | Medium-firm to firm | — |
| NCOA, back pain | Nectar Premier Hybrid, NCOA’s best overall | Category pick: Luxe with ErgoAlign, NCOA’s best for lower back pain | Medium-firm | Queen range $1,249–$2,375 |
| Naplab, back pain | WinkBed, Naplab’s top pick | Category pick: Luxe, Naplab’s best overall with pillow top | 6 to 7 out of 10 | WinkBed $1,799; Helix Midnight Luxe $1,899 |
A dash means the figure isn’t carried in this table, so check that page for it.
The tally: a Helix Midnight model comes first on 4 of 6 lists and appears on all six. Counted by publisher, that’s 4 of 5, but only by counting NCOA’s orthopedic list, which covers a different scope. On NCOA’s back-pain list, the Luxe is a category pick. Naplab is the odd one out, with the WinkBed first.
AARP’s Smart Picks team reviewed over 50 mattresses, most in queen size, before naming the Helix Midnight Luxe.
Where the Lists Differ: Model, Layer and First Pick
- Model. Three lists name the Luxe. Sleep Advisor names the Helix Midnight with the ErgoAlign layer instead. Read that row as the same line, not the identical bed.
- Layer. Mattress Clarity’s winner is the Luxe “with the optional ErgoAlign layer”. NCOA’s back-pain list names the Luxe “with ErgoAlign”.
- First pick. NCOA’s back-pain list puts the Nectar Premier Hybrid first, and Naplab puts the WinkBed first.
- Firmness. Naplab gives a number, 6 to 7 out of 10. Sleep Advisor and NCOA give words: medium-firm to firm, and medium-firm.
What These Beds Cost at Queen Size: $1,249 to $2,375
The beds on NCOA’s back-pain list cost $1,249 to $2,375 for a queen (NCOA). Its orthopedic list covers a wider range, $998 to $3,499 in queen size.
Naplab lists the Helix Midnight Luxe at $1,899 and the WinkBed at $1,799 (as listed Oct 2026). That’s $1,899 − $1,799 = $100 more for the Luxe, about 5.6% ($100 ÷ $1,799), per Naplab.
Verdict: Shortlist the Helix Midnight Luxe, Priced With Its Add-On Layer
- Verdict: The Helix Midnight Luxe is the model these rankings most often put first for back pain, so it’s a sensible first name on your shortlist.
- The flaw, and its fix: The lower-back layer behind Mattress Clarity’s pick is optional. Its winner is the Luxe “with the optional ErgoAlign layer” (Mattress Clarity), so a base order may not include it. Fix: add the ErgoAlign layer at checkout, then compare that price with NCOA’s $1,249 to $2,375 queen back-pain range.
- Strengths: With ErgoAlign, it has “a layer specifically designed to support the lower back”, per Mattress Clarity. NCOA’s orthopedic list credits it with “excellent back support and pressure relief in every position” (NCOA). Naplab rates it Medium-Firm (6), inside its 6-to-7 back-pain range.
- Who should skip: heavier sleepers → the WinkBed Plus, Naplab’s pick for heavy people (Naplab), or the Titan Plus, NCOA’s pick for higher-weight people. Side sleepers → the Nectar Premier Memory Foam, per NCOA. If price matters most → the Birch, Naplab’s affordable natural-materials pick. Naplab lists it at $1,499, $400 below the Luxe (Oct 2026).
Who Pays the Reviewers: Four of Five Publishers Disclose Commissions
Four of the five publishers say they earn money from their links. NCOA says it earns affiliate commissions on its back-pain list, and its orthopedic list carries a disclosure too. Naplab says it may earn a commission from its links.
Mattress Clarity says it gets free products and joins affiliate programs. Sleep Advisor says it may receive a referral fee.
A commission doesn’t make a pick wrong. It is a reason to trust where the lists overlap more than any single list. Before you click a buy link, do one check: open the maker’s checkout and confirm the queen price for the same model and layers the ranking reviewed. If the two prices differ, go by the checkout price.
Next step: Compare pillows and other sleep gear on the sleep and mattresses hub.
FAQ
Is a firm mattress better for back pain?
Sleep Advisor recommends medium-firm to firm. Naplab’s range is 6 to 7 out of 10, and NCOA’s back-pain list focuses on medium-firm models. The aim is a level spine. Firmness is how you get there.
Does “orthopedic” on a label mean a bed meets a standard?
No. NCOA notes the term has no formal definition, so read it as a description of the design.
Will a new mattress stop my back pain?
No source here promises that. Mattress Clarity calls a poor mattress a possible large contributor among many causes. A new bed may help some people and do little for others.
Which costs less, the WinkBed or the Helix Midnight Luxe?
Naplab lists the WinkBed at $1,799 and the Helix Midnight Luxe at $1,899 (as listed Oct 2026), a $100 gap.
